1) Narcissism is a product of karma, but yes, it also creates more karma. If you deliberately ignore the needs of other people in one life, you're likely to end up being unable to notice those needs in another life, and heaping up karmic pain as a result.
2) You have to be very, very far along the path of inner growth to choose anything in your next life. No, as just noted, being a narcissist is a karmic penalty for bad behavior in a previous life.
3) I think a lot of it is the collective karma of our civilization, which did a lot of riding roughshod over everyone else. That's going to be reflected in the behavior, and thus in the karma, of people who can't or won't turn a reflective eye on their own society and refuse to participate in its follies.
