I know in some parts of Vedic astrology they make a distinction between four different elements of birth time, so that the "fire" birth time corresponds to first emergence, the "air" birth time corresponds to the first breath, the "earth" birth time corresponds to final emergence, and the "water" birth time corresponds to the severance of the umbilical cord:

Conceivably, someone who is born with an umbilical strangulation that isn't resolved quickly could have an unusual skew in their birth time. (Though in Vedic astrology a nuchal cord is considered a separate indicator of, besides a bunch of technical stuff I don't follow, heightened malefic power of the lunar nodes.)
