That's the end price paid by the patient. It's actually worse than that. When I first started working in pharmacy, this would have been around 1997, I once happened to see an order invoice that was laying on the accountant's stack of paperwork.

It was for the generic of Tylenol #3, that is, regular strength tylenol with 30mg of codeine added- a common opiate painkiller for after dental surgery. The *wholesale* cost listed: $0.02 per tablet. Yes, you read that right- 2 cents per tablet. The 'standard formula' charge to the patient? Five dollars per tablet.
