I myself am slowly trying to reverse engineer a "lodge" for me to be a doctor to. I have established a set of clinic rates (I practice TCM acupuncture) that are extremely reasonable to "members", and I am slowly acclimatising the "members" to the larger benefits of being members, by adding other member percs. In the coming year I will start holding "member" events to allow people to begin to get to know each other. Hopefully, within 10 years or so, there will be a full-fledged lodge which can add other medical practitioners, and general health services to the portfolio it offers its members.
