I recently had to switch ophthalmologists because the old one retired, and I had to do new patient intake forms. They asked about the flu and cooties jabs. Not sure why. I left the answers blank, because I wanted to see if they'd push me to answer so I could ask, "why do you need to know?". But they didn't ask.
