1 & 2) I've worked with many different versions of the SoP and never noticed any trouble at all, however it was done. Thus I'd encourage you to -- wait for it -- "try the different versions and see for yourself." ;-)

3) I wish! I learned it directly from John Gilbert, who invented it based on a simpler ritual that was devised by Juliet Ashley. I've never encountered it anywhere else.
