The worst thing in all of this is the us v. them dynamic where the doctor/nurse/pharmacist/computer coders/administrators/insurers side constantly closes ranks to hide errors, both the innocent and gross negligence - and gets away with it thanks to uneducated patients and their families who all too often meekly "trust the experts."
