Hmm this is an interesting problem. I've spent a few minutes searching for e-readers/pdf readers without internet connection and can't find anything. I use an old Kindle Paperwhite that almost always stays in airplane mode which I acquired before I soured on the "river company". I know that doesn't check all your boxes but I'm sure you could find one used if nothing else turns up. It does provide a nice reading experience for most pdf's.
