The obvious worry would be some sort of hemorrhagic fever, so why weren't the passengers quarantined?
Realistically, the guy probably had esophageal varicies which catastrophically ruptured (they can do that, and chronic alcohol abuse can lead to that). I gather he wasn't bleeding out of his eyes as well, which hemorrhagic fevers do and esophageal bleeds don't, but I'd sure would have like to have seen some due diligence on the part of public health authorities to ensure they weren't spreading ebola or Marburg fever all over the world.
